Worker Compensation

Firefighters wishing to file a claim should notify the department chief immediately for assistance with the appropriate forms. If the chief is not available, contact the assistant chief and then the chief medical officer.

• For each payable, please send email to both CHIEF@SLFPD.ORG and TREASURER@SLFPD.ORG

• SUBJECT: PAYABLE TO | AMOUNT (i.e. Fred's Propane 249.49 or Jane Doe 179.75)

• Attach a picture of the bill, invoice, expense reimbursement or receipt. IF POSSIBLE, please use the following file name conventions - DATE YYYYMMDD VENDOR AMOUNT (i.e. 20220515 FredsPropane 249.49) so we can file the pictures in our accounting AND digital filing system for easy retrieval if needed. If you’re being reimbursed then your name goes in the VENDOR spot.

• Once submitted, Chief will review and REPLY TO ALL with approval and coding for payment processing. If you do not receive a confirmation email within 5 business days, please re-send to both of us.

• Lastly, due to month end closing processes, we will process all payables on the 10th and 20th of each month to be paid by the 15th and 25th respectively. These payments will be via Melio, which is an approved QuickBooks Online payment processor and can be made via ACH in 2-3 days if you have setup your bank account information. If not, payments will be sent via check to your USPS mailing address on file and may take up to 7 more days.
